Documents usually required
- ✓Proof of premises: ownership papers or registered rent agreement
- ✓Premises layout plan / key plan showing area (minimum 10 sq. m)
- ✓Registered pharmacist's MSPC registration certificate
- ✓Pharmacist's appointment letter and affidavit
- ✓Proprietor / partners / directors ID and address proof
- ✓Partnership deed or MOA/AOA (if not proprietorship)
- ✓Shop photographs (outside and inside), with refrigerator for cold-chain medicines
- ✓Fee challan

Frequently asked questions
Do I need to be a pharmacist myself?+
No. The owner need not be a pharmacist, but a registered pharmacist must be employed at the store during working hours.
What is the minimum shop area?+
Generally 10 square metres for a retail store, and 15 square metres for combined retail and wholesale. We check your premises before applying.
